In the POSITIF study, people who had experienced a stroke were willing to be randomized, although treatment adherence was lower than the authors had hoped.1 Along with increasing adherence, the authors speculated that the intervention might have been more effective had it been delivered by staff who were already experienced delivering cognitive behavioural therapy.1 Further work, particularly around intervention design and testing, is needed to understand post-stroke fatigue.1
