The Alcohol Dependence Scale (ADS) was designed to measure the severity of alcohol dependence across 25 items.[1] The ADS is a widely-used, quick to use, self-administered questionnaire.[1] Yes/no items are scored as 0–1; three-choice items as 0–2, and four-choice items as 0–3, with higher scores indicating greater dependence.[2]

The ADS Total score ranges from 0 (no evidence of alcohol dependence) to 47 (severe level of alcohol dependence),[2] with a score of ≥9 highly predictive of a diagnosis of alcohol dependence.[1]
